In our commitment to meet the unique needs of the veterans we serve, Yolo Hospice is a partner of We Honor Veterans, a program of the National Hospice and Palliative Care Organization (NHPCO) in collaboration with the Department of Veterans Affairs (VA). We recently achieved a new status as a 4-Star Partner of We Honor Veterans.
This is the highest level of recognition in this program, and reaching it has involved years of team effort to improve our services for veterans and their families. We are grateful to all the team members who helped Yolo Hospice reach this point, and also want to express gratitude to the Veteran Services officers who have worked with us, especially Billy Wagster of Yolo County and Ted Puntillo of Solano County.
Our 4-star partner status is based on meeting a series of criteria in how we serve veterans, including educating our staff on the unique issues in supporting veterans at end-of-life; instituting procedures for assisting veterans and their families to receive benefits; and developing relationships with local VA offices. Yolo Hospice is committed to serving the veterans who have done so much to serve our country, assessing and meeting their unique concerns to give each patient support at end of life.
This achievement marks the next stage in our ongoing dedication to serve our veteran patients. Our patient assessments have shown that, at any given time, between one-third and one-half of our patients are veterans. We will continue our commitment to serving veterans and their families with the highest level of care.
