When curative treatment is no longer achieving its intended objectives for your patient, it may be time for a transition to new goals, and to actively pursue comfort care. Early access to palliative care provides many benefits, including managing patients’ symptoms, providing social and emotional support, and providing clarity for decision making. The right support can improve the quality of care and reduce the use of medical services.
Our Yolo Hospice team is available to you 24-hours a day to support your patients, their loved ones, and their caregivers; and to provide individualized support and symptom management for your patient’s physical, emotional, and spiritual needs.
We are dedicated to assisting you to make your patient and their loved ones comfortable, educating them and helping to manage their expectations about end-of-life. You or your staff may refer a patient at any time, or contact us to discuss your patient by calling (800) 491-7711.
