By discussing end-of-life care and final arrangements, you can save yourself stress and give your loved ones a gift—the gift of knowing your preferences.
Discussing and recording one’s wishes, including completion of advance directive forms, is important so that you may live your life, especially your end-of-life, on your own terms.

Five Wishes
Yolo Hospice provides an Advance Directive workbook, Five Wishes, free of charge to anyone in the community. The workbook is designed to help you specify how you want to be medically treated if you have an accident or life-limiting illness. It also allows you to name the person you want to make healthcare decisions for you if you become unable to make them yourself. This easy-to-complete form is valid in 40 states, including California, and does not require the assistance of an attorney. It can be updated or changed at any time. Five Wishes is not just for seniors — it is for anyone 18 or older.
